Virtual care is often described as lightweight, flexible, and digital-first.

  • Care anywhere.

  • Care without walls.

  • Care untethered from physical spaces

But in practice, the success of virtual care programs depends on something far less abstract: real-world infrastructure. Devices still need to arrive at patients’ homes. Connectivity still needs to work. Data still needs to flow reliably. And support systems still need to function outside the clinic.

Virtual care is a physical infrastructure distributed across thousands of patient homes.

The Myth of “Virtual” Care

The term “virtual care” creates a misleading mental model.

It suggests that care delivery happens primarily through software with video visits, dashboards, platforms, and apps. That belief shapes how many programs are designed, funded, and scaled.

Early on, this works. Small cohorts are manageable. Exceptions are rare. Teams compensate with manual processes. But as programs grow, the gap between perception and reality becomes harder to ignore.

What Infrastructure Really Means in Care Delivery

In traditional care settings, infrastructure is obvious:

When these elements work together, care feels seamless.

The Patient Home Is a Care Site

Virtual care effectively turns every patient's home into a point of care. That shift carries implications that many teams underestimate.

Unlike clinics, patient homes:

  • Vary widely in environment and connectivity

  • Require devices to work out of the box

  • Depend on clear onboarding and support

  • Lack of on-site staff to troubleshoot issues

This means infrastructure can’t just exist. It has to be reliable, repeatable, and resilient.

If a device arrives late, care is delayed. If it fails to activate, monitoring stalls. If data is unreliable, clinicians lose trust.​

The home may be remote, but the expectations for care delivery are not.

Where Teams Underestimate Complexity

Most virtual care programs don’t struggle because of ambition. They struggle because early success hides complexity.

Common blind spots include:

  • Treating device fulfillment like e-commerce

  • Managing logistics separately from clinical workflows

  • Assuming onboarding scales linearly

  • Deferring visibility until “later”

  • Underplanning for retrieval and redeployment

Each decision feels reasonable in isolation. Together, they create friction that compounds as volume grows.

What starts as an operational inconvenience eventually shapes patient experience, clinician confidence, and program adoption.

How Infrastructure Choices Shape Adoption

Adoption is often framed as a clinical or behavioral challenge. In reality, infrastructure plays a defining role.

Clinicians are more likely to trust virtual care when:

  • Devices arrive on time

  • Data is consistent and reliable

  • Exceptions are handled quickly

  • Operational issues don’t disrupt workflows

Patients are more likely to stay engaged when:

  • Setup is simple

  • Support is accessible

  • Technology works without friction

In both cases, adoption isn’t driven by novelty. It’s driven by reliability.

Programs that treat infrastructure as a strategic priority tend to scale with fewer disruptions and greater confidence, even as complexity increases.

Infrastructure Is the Foundation of Care Anywhere

Virtual care expands where care can happen, but it doesn’t eliminate the need for infrastructure. It raises the bar.​

Care anywhere requires:

  • Devices designed for lifecycle reuse

  • Logistics aligned with clinical timelines

  • Visibility across patients, inventory, and data

  • Systems that support scale, not just pilots

When infrastructure is treated as foundational rather than supplemental, virtual care becomes dependable. And that dependability is what ultimately enables care anywhere to work in the real world.
